Thank you for your interest in the Alaskan Klee Kai and Aurora Kennels. Please complete the following questions and return this form to me by email, (cut and paste into either word or a new email) so I can better determine if the Alaskan Klee Kai might make a pleasant addition to your household. All puppies and adult dogs will be placed where they will be loved and be happy, and it is just as important that you also be happy with my dogs. As we all know, there is no one breed of dog that is the right breed for everybody, and Alaskan Klee Kai is no exception. When you bring an AKK into your family, you are making a commitment to raise, train and care for your dog throughout its lifetime. Alaskan Klee Kai are intelligent and active dogs that require training, exercise and socialization to become welcome additions to your life. Puppy socialization, training and obedience classes are highly recommended. This questionnaire is to help you think through the factors involved in becoming a puppy owner and for me to best make a decision regarding placement of my pups.
This questionnaire will become a legal part of the Sale Contract in the event you decide to obtain a puppy from Aurora Kennels/Diana Hamster.

· I agree to keep this dog in my possession and will not give away, sell to any person, pet store or puppy broker, or abandon it - I will return it to the breeder if I relinquish custody.
· I agree to provide a safe, clean environment with a securely fenced yard.
· I agree to provide a daily routine of feeding, training, exercise, play, and nurturing.
· I agree to care for this dog humanely at all times, providing necessary medical care and regular check-ups.
· I agree to make every effort to put a title on the dog IF purchased for breeding.
· I understand these conditions may not be waived without the written consent of the breeder.
